New Patient Registration

In order to register with the practice you should bring your NHS medical card and 2 items with proof of address e.g. utilty bills, telephone bills or bank statements dated within the last 3 months. If you do not have a medical card you will be asked to fill in a form and questionnaire. As a guide, our practice area serves Pollok and surrounding areas which includes G51, G52, G53, parts of G41, G43, G46 as well as some of PA1 and PA2.

If you prefer a particular type of doctor, e.g. female for a female patient and vice versa we will note this and do our best to respect your choice. However, not all the doctors in the practice provide all services and specific doctors may not be immediately available. 

It is important that we meet you as soon as possible to discuss your medical history and medication, and assess your health needs. Please remember it may take several weeks for your case notes to be transferred from your previous doctor.

Medical treatment is available from the date of registration. Please contact reception for further information.
